- Please fill out the form for EACH individual practice missed and wanting to make-up
- Coaches who are looking for extra training players for their practices will use this form and contact YOU if they are needing extra players for their practice
- It is NOT guaranteed that coaches can help make-up your missed practice, it's very important that coaches are able prioritize their focus on their respective team members first and then help facilitate to those who need to make up a practice secondary.
- It DOES help to follow up with your specific Head Coach as they can also facilitate conversations with other Head Coaches from other teams about trying to help your athlete makeup practice dates.
